Achieving the Ideal Placement

UNDERSTANDING THE CHALLENGES

There comes a time when families and friends become concerned for a loved one due to the possibilities of falling, getting sick, safety issues, forgetting to take medications or difficulty taking care of household responsibilities. It is important to recognize these signs early and work with your loved one to determine the appropriate living situation.

There is a broad array of housing options available, from staying in your own home to specialized facilities that provide round-the-clock care. To begin planning for a new living situation, one must first be aware of the different options available and to learn which type will best suit your loved one’s needs.

When a family is faced with having to make a decision about the care and living arrangements of a loved one, it’s a very difficult time. Our role helps families find the appropriate arrangements. Two Caring Hands offers experience, knowledge and compassion to walk with you through this process. It is important that we find the right solution the first time. We will help you navigate through the entire process from start to finish.

EMBRACING THE PROCESS

Step 1: Family Meeting

We begin with a personal meeting to become familiar with you and your loved one’s needs and desires. You may ask us questions and help us to understand lifestyle preferences. Next, we check the availability of your preferred locations. We will only recommend accommodations that we have viewed in person, fully researched and which meet our high standards. We will only recommend accommodations which meet your needs and desires.

Step 2: Touring Your Preferred Options

We schedule tours of your top three or four options and will gladly accompany you on the tours. This is one of the important ways that we are unique from other services. We will take notes, share our observations, and explain options. During the tours, we will take notes and assist with the questioning and inquiry process and assist with the comparison of details from each community.

Step 3: Calculating the Cost, Relocation, and Follow Up Services

A medical assessment will be done to determine the level of care and cost of care. We will continue to assist you in relocating your loved one. We will conduct a follow-up visit within one month of placement to ensure it is still the right fit.

Once you’ve decided on a new home, we can help you with other optional services you might require, such as:

  • Real Estate Services
  • Moving Services
  • Cleaning Services
  • In-Home Care agencies
  • Estate Services
  • Funeral Service Professionals
